COVID-19 testing in the United States is currently at inadequate levels, meaning both the public, healthcare officials, and public health officials are unsure the true current spread of the coronavirus pandemic.

We are currently asking people to self-report any COVID19-like symptoms to get a more accurate representation of the spread.

Personally identifying information will not be made public. CrowdSource Rescue is primarily a disaster response organization; this is a citizen science initiative for the benefit of the public.

This shouldn't need to be said, but this is OBVIOUSLY not a replacement for medical professionals. If you suspect you have COVID-19, go to a medical professional first. This form is only for people who have been diagnosed with COVID-19 or whose physicians suspected COVID-19 (but were unable to procure testing).
